Act 24, 34, 114, 151 Clearances

In accordance with PA House Bill 435 which has now been signed into law by the Governor (Act 153) and pursuant to Section 6344.4 relating to Certification Compliance, WHSD's policy on Employee & Volunteer Clearances are as follows (clearances will need resubmitted EVERY 5 years going forward):
Please print and read this form and submit the signature page only to the District Office. Act 24 requires all current school employees and volunteers to complete a Pennsylvania Department of Education (“PDE") form to report prior arrests or convictions for any offense listed in the newly expanded Section 111(e).  If you have committed any of the reportable offenses listed on the 6004 form since your last clearance date, you have 72 hours to submit a new 6004 form.
Please complete and pay for the on-line form ($22) at If you already have completed an Act 34 clearance form within the last 12 months, it is not necessary to complete another one. Submit a copy with your application. If you are contacted for an interview, you must bring the original form with you. If you are a volunteer - there is no charge for this clearance.
You must provide a copy of the Pennsylvania Child Abuse History Clearance from the Department of Human Services (formerly Dept. of Public Welfare). This form ($13) may be completed on-line at Check the appropriate box: "School Employment" block if you will be an employee, or the "Volunteer" box (for volunteers, this clearance is free). Once you are notified the clearance has been processed, print a copy of the certificate and send a copy to the District Office.
Act 114 Fingerprinting Application (FBI Clearance: Volunteers who have lived in PA less than 10 consecutive years will need to submit this clearance - as well as ALL applicants for school district employment). The Service Code for PAID employees is 1KG6XN.  Registration and payment is done online. Take the registration page with you to get your fingerprints taken an authorized IdentoGO fingerprint location. By sending the District Office a copy of your FBI registration page, we can check the results on-line. You will be mailed a letter from the FBI with the results.
Please keep a copy of all clearances, as it is your responsibility to have them updated every five years.
